Thresholds provides essential mental health and substance use services across the South Side. In the 9th District—encompassing Brighton Park, Gage Park, Back of the Yards, New City, Canaryville, and Englewood—they focus on meeting residents where they are, whether in a clinical setting or through street outreach.
Thresholds uses a centralized intake process to ensure residents are matched with the right level of care, whether they need outpatient therapy, housing support, or intensive outreach.
-
Step 1: Central Intake Call: The first step is to call the Central Intake line at (773) 572-5500.
-
Step 2: Information Gathering: Staff will ask for basic information, including the specific needs of the individual and insurance status (Thresholds works heavily with Medicaid).
-
Step 3: Referral & Connection: Once screened, the individual is referred to the nearest appropriate team. For 9th District residents, this often means a connection to the Thresholds Bridge Southwest 3638 S Kedzie Ave, Chicago, IL 60632
Administrative & Intake Hours
While outreach teams operate with flexible hours to meet community needs, the administrative and intake operations generally follow these standard business hours:
-
Monday – Friday: 8:30 AM – 5:00 PM
-
Saturday – Sunday: Closed
LIVE FREE Chicago is a faith-based community organizing initiative dedicated to ending gun violence, mass incarceration, and the criminalization of Black communities. -
Decarceration & Reentry: Their "Decarceration Fellowship" supports individuals returning home after imprisonment, helping them navigate the challenges of reentry and providing a community that understands their journey.
